Pay Mechanism: Direct Engagement

Job Overview:

We are seeking a skilled and motivated Band 6 Occupational Therapist to join our community health team. This role will focus on providing holistic and patient-centered care in various community settings, supporting individuals with complex rehabilitation needs to achieve independence and improve their quality of life.

Key Responsibilities:

  • Conduct assessments of patients in their home or community setting, developing personalized care and rehabilitation plans.
  • Deliver high-quality interventions to support patients' mobility, daily living skills, and mental wellbeing.
  • Coordinate with multidisciplinary teams, including physiotherapists, nurses, and social care professionals, to ensure comprehensive care.
  • Support and educate patients, families, and caregivers on rehabilitation techniques, equipment use, and safety practices.
  • Contribute to discharge planning, ensuring patients can safely transition back to their home environment with appropriate support.

Requirements:

  • HCPC registration as an Occupational Therapist.
  • Experience in community healthcare or relevant clinical settings.
  • Strong assessment, planning, and rehabilitation skills with a patient-centered approach.
  • Excellent communication and teamwork abilities, especially in multi-agency collaboration.
  • Ability to work independently and adapt to varied community environments.
