Greater Manchester Mental Health are currently looking for an experienced Mental Health Practitioner (Social Worker, RMN or Occupational Therapist) to join their Early Intervention Team at Cromwell House. Within this role, you will provide a person centred, statutory social work service for a caseload of patients in accordance with relevant policies and procedures. You will be assessing and planning support plans to meet individual needs as well as working with partners to safeguard service users in the community This service supports people presenting with a first episode Psychosis.
What you can expect:
- Management of discharged from the service and transfers to other services
- Working collaboratively to seek resolutions to challenges
- Completing specialist assessments for all referred service users
- Finding strategies to meet your patient's goals
What you'll need:
- Degree in Social Work or equivalent qualification
- HCPC Registration
- Experience of conducting assessments identifying health and social care needs
- Relevant specialist training in the skills required for the specialty area
In addition to experience, all Qualified roles will be required to provide evidence of:
