Job Summary:
- To provide a specialist response in complex cases
- Formulating and implementing intervention plans aimed at reducing the impact of disability and ill health
- Making onward referrals to the Community Occupational Therapy team where complex fixed equipment or major adaptations are required
- To assess an individuals occupational performance and functional needs using standardized and non-standardized tools
Special Requirement: Must be able to drive and own a vehicle for work.
