Your Role:
- Coordinate discharge planning for patients.
- Work with multidisciplinary teams on individualised plans.
- Communicate with patients and families about post-discharge care.
- Ensure compliance with hospital policies.
Requirements:
- Degree in social care
- Social Work England Registration
- Experience in Hospital Discharge team
