Job Description:
Are you an experienced social worker looking to make a difference in a community setting?
In this role, you will work with individuals and families to assess needs, develop care plans, and provide ongoing support to help them achieve their goals and improve their quality of life.
Responsibilities:
- Conducting assessments to identify the needs and strengths of individuals and families in the community.
- Developing and implementing care plans that focus on achieving positive outcomes and promoting independence.
- Providing ongoing support and intervention to individuals and families, working to address their unique needs and challenges.
- Collaborating with multi-disciplinary teams, external agencies, and community organizations to coordinate care and services.
- Monitoring progress, reviewing care plans, and making necessary adjustments to support long-term goals.
Qualifications:
- Qualified Social Worker registered with Social Work England.
- Experience in a social work role, particularly in a community or locality setting.
- Strong assessment, planning, and intervention skills, with a focus on promoting independence and positive outcomes.
- Knowledge of relevant legislation, policies, and procedures related to social care.
- Ability to manage a diverse caseload and work collaboratively with a range of professionals.
