This is a full time position working 37.5hrs/week over a 5 day week
Job description:
- Community Rehab Occupational Therapist to facilitate supported discharges home from hospital, condition management, falls assessments, activity of daily living assessments, interventions and reviews.
- Weekend working may be required.
- The role will involve a holistic approach to encompassing physical rehabilitation and refer to wider community services.
- You will improve experience for patients and their families. We promote continuity of care for patients in the community setting.
