Support Worker

Reed are working with a globally recognised organisation to recruit a dedicated Support Worker to join the team based out of East Surrey Hospital in Redhill. The role will help the organisation to provide essential assistance to service users in the East Surrey area who have recently had a spell in hospital, enabling them to return to their own homes and avoid hospital re-admission.

This role involves responding to referrals from clinicians and community health professionals, assessing service users' needs, and developing support plans to promote independent living.

Support Worker, Assisted Discharge Service

  • £11.44/hr plus holiday pay
  • Mileage paid
  • East Surrey
  • Part-Time, 21hrs/week, NO WEEKENDS

Day to Day of the Role:

  • Assist individuals returning from hospital or at risk of health decline, ensuring their safe transition back to their homes.
  • Accompany service users to hospital appointments and other activities as outlined in their care/support plan.
  • Conduct ongoing assessments of service users' needs and collaborate with them to create a tailored support plan.
  • Monitor and report on service users' conditions, taking appropriate action if health deteriorates.
  • Liaise with health, housing, and other professionals to ensure consistent support for service users.
  • Perform risk assessments to ensure a safe living environment, respecting service users' dignity, choice, and rights.
  • Conduct follow-up welfare checks, visits, and assist with activities to re-enable daily living skills.
  • Provide practical and emotional support, including companionship, shopping assistance, and signposting to further support services.

Required Skills & Qualifications:

  • Experience in a S upport Worker role or similar position.
  • Ability to assess needs and develop individual support plans.
  • Knowledge of health and safety legislation and risk assessments.
  • Understanding of safeguarding policies and procedures.
  • Strong communication and interpersonal skills.
  • Commitment to the values of inclusivity, compassion, courage, and dynamism.
  • Driver's Licence (Manual car) is essential for this role
