£25.00-£26.50 (Paid weekly via an umbrella)
Job Role
To provide high standards of care to patients with highly complex continuing care needs. Care will be delivered within the patient's own home / other community setting or local hospital. The post holder will be required to deliver care independently but will be expected to report back to other members of the team, the patient and family. The post will involve lone working; there is always a qualified nurse on-call for support.
- To maintain confidentiality on all matters relating to patients/families adhering to Trust policies.
- Assess patient care needs. To implement and evaluate programmes of care with patients and their families.
- Ensure that the delivery and quality of care given complies with local and Trust agreed policies and procedures.
- Communicates effectively with patients/families/other members of the multi-disciplinary team to ensure prescribed plan of care is delivered.
- To ensure that patients receive high standards of personal care.
- To keep accurate and legible patient records adhering to Trust policy.
- Works in patient's homes without direct support in complex circumstances, maintaining professional boundaries and integrity at all times adhering to the local Code of Conduct.
Essential
- Experience working with children, and/or nurses with ITU experience
- Management of tracheostomies, ventilators and complex seizures.
- Driving Licence essential
