Mental Health Social Worker Hybrid

Requirements:

  • Social Work England Registration: Must be a registered Social Worker with Social Work England.
  • Experience: Extensive experience in Mental Health Social Work, including conducting Mental Health Capacity Act assessments and working with adults with mental health needs.
  • Enhanced DBS: A valid and current Enhanced Disclosure and Barring Service (DBS) check.
  • Skills: Strong assessment, planning, and reviewing skills, with the ability to apply a strength-based approach in practice. Excellent communication and interpersonal skills are essential for working remotely.

**Key Responsibilities:

  • Mental Health Assessments: Conduct initial and comprehensive mental health assessments for adults with less complex mental health needs. Determine eligibility for services in accordance with the Mental Health Capacity Act.
  • Care Planning and Reviews: Develop, implement, and review care plans in collaboration with service users, their families, and other professionals. Ensure that care plans are strength-based, person-centered, and promote independence.
  • Collaboration: Work closely with other members of the Mental Health Reviews Team, healthcare professionals, and external agencies to provide holistic care to service users. Facilitate multi-disciplinary meetings to support decision-making processes.
  • Mental Capacity Act Compliance: Ensure that all interventions comply with the Mental Health Capacity Act, including assessments of capacity and best interest decision-making.
  • Safeguarding: Identify and respond to safeguarding concerns, ensuring that adults at risk are protected and supported effectively.
  • Documentation and Reporting: Maintain accurate, timely, and detailed records of all assessments, reviews, and interventions. Provide reports and evidence for legal processes when necessary.
