As a Diabetes Nurse Specialist, you'll play a vital role in improving outcomes for people with Type 2 Diabetes. You'll work in line with national guidelines and use your clinical expertise to deliver personalised care, optimise treatment, and support education and empowerment for both patients and primary care teams.
If you're a Nurse with at least 3 years of experience managing Diabetes in primary care, and you're ready to work autonomously in a role that makes a measurable difference, we'd love to hear from you. A Nurse progression pathway and full support and funding for diploma-level qualifications will be on offer to help you thrive and grow in a rewarding specialist role.
Key Responsibilities for the Diabetes Nurse Advisor:
Identify patients with sub-optimally controlled Type 2 diabetes in primary care.
Optimise oral hypoglycaemic therapy in line with national guidelines.
Deliver patient-centred education, support, and advice.
Conduct point-of-care testing and refer patients for specialist insulin treatment where needed.
Work collaboratively with GP practices while maintaining corporate nursing responsibilities.
Travel to and from clinic locations, with typical working hours of 8:30am - 5:30pm (6-hour clinics + travel time; maximum travel advised is 2 hours each way).
Requirements for the Diabetes Nurse Advisor:
Diabetes diploma or equivalent qualification.
Minimum 3 years' post-registration experience managing Diabetes in primary care.
Confident and knowledgeable in educating and advising people with Type 2 Diabetes and optimising oral hypoglycaemics.
Full UK driving licence.
NMC registration.
Competent IT skills, including the use of primary care systems.
Strong interest and knowledge in Diabetes management.
Salary and Package on offer:
Salary ranges from £38,000 to £44,00 (FTE), in line with experience and qualifications
