You will deliver high quality nursing care for patients who have been discharged from hospital following a stroke, related to their individual goals and needs. The post holder will receive support and supervision from the Band 7 Stroke Nurse Team Lead. The post holder will be able to work closely with Occupational Therapists, Physiotherapists, Speech and Language Therapists, Psychologists and Therapy Assistants. We are looking for an NMC Registered Nurse who has excellent communication skills as well as a strong interest and experience in stroke and rehabilitation.
Main duties of the job
You will work closely with other Nurses to complete holistic nursing assessments as well as plan, deliver and evaluate evidence-based interventions to achieve the best outcome for patients.
Develop knowledge and clinical competence within stroke and rehabilitation and utilise this to deliver excellent clinical care with the support of Senior Nurses and the Multi-disciplinary Team.
Use excellent communication skills to engage and motivate Stroke Survivors to engage in rehabilitation and adhere to secondary Stroke prevention advice, as well as liaise with relevant professionals within the team and in wider services such as GP practices, Stroke Consultants and the Stroke Association
This role involves seeing patients in their own homes and will require you to be a car driver and to see patients independently following appropriate induction
To complete relevant administrative duties such as accurate and concise documentation of notes, reports and data recording as required by the role.
You will play a role in co-ordinating referrals establishing suitability for the service and attending hospital multidisciplinary meetings as required.
You work as part of an interdisciplinary team who treat all patients as individuals with a goal focused holistic approach.
