Job Description:
Are you a skilled social worker with a passion for making a difference in the lives of disabled children and their families? We are seeking a motivated individual to join the Disabled Children's Team in Stockton on Tees.
Responsibilities:
- Conducting thorough assessments of the needs and circumstances of disabled children and their families.
- Developing and implementing individualized care plans that address the specific needs and goals of each child.
- Providing support and advocacy for disabled children and their families, ensuring they have access to necessary services and resources.
- Collaborating with healthcare professionals, educational staff, and other relevant agencies..
- Maintaining accurate and detailed records of assessments, care plans, and interventions in line with legal and professional standards.
Qualifications:
- Qualified Social Worker registered with Social Work England.
- Experience working with disabled children and their families, preferably within a social care setting.
- Strong assessment, planning, and intervention skills.
- Excellent communication and interpersonal skills, with the ability to build trusting relationships with children and families.
- Knowledge of relevant legislation, policies, and procedures related to disability and child welfare.
