Physical Health Nurse

What you will be doing:

As a qualified Physical Health Nurse, you will lead and coordinate all aspects of the physical health needs of service users, while also supporting your colleagues and promoting good teamwork with the multidisciplinary team, such as GPs and other physical health departments.

You will be responsible for monitoring observations and assessments to identify any actions that need to be addressed and documented.

As a Physical Healthcare Nurse, you will be:

  • Adhering to the NMC Code of Conduct and facilitate the compliance with local, Trust and national standards of practice.
  • Developing and supporting physical health interventions used in the clinical area using knowledge of current best evidence-based practice.
  • Engaging actively in practice development, evaluation, audit activities relevant to physical healthcare and/or service areas to promote evidence-based practice.
  • Working collaboratively with primary health, developing and maintaining links with GP practices, and other physical health departments.
  • Monitoring and maintaining Physical Health Dashboards.

Where you will be working:

Location: The Farndon Unit, Farndon Road, Newark, Nottinghamshire, NG24 4SW

You will be working at the Farndon Unit, a purpose-built low secure hospital which offers personalised assessment and recovery-based treatment pathways, including a trauma specific pathway, for women with complex care needs.

You will join the multidisciplinary team in providing specialist care for people with a diagnosis of severe and enduring mental illness and/or personality disorder and/or mild learning disability.

The combination of general low secure wards and a recovery ward area (also within the low secure perimeter) provide a seamless internal care pathway supporting and empowering recovery and rehabilitation.
