Job Summary:
A loyal client of ours, based in North Wales is seeking a dedicated and compassionate Qualified Social Worker to join their Children with Disabilities (CWD) team. This role provides an opportunity to support children with disabilities and their families, ensuring they receive high-quality services and the necessary interventions to thrive in a nurturing environment.
Key Responsibilities:
- Comprehensive Assessments: Conduct thorough assessments of children with disabilities to develop tailored care plans in collaboration with families and other professionals.
- Therapeutic Support: Apply trauma-informed practices to support children affected by complex backgrounds, ensuring that their emotional and developmental needs are addressed.
- Care Planning & Court Reports: Manage a caseload of 6-8 cases, including court reviews and legal documentation, as well as 16-20 cases for long-term visitation (every six months), and a mix of looked-after children (LAC) cases.
- Multi-Agency Collaboration: Work alongside health services, education, and other external agencies to ensure seamless coordination of support for each child.
- Duty Day Commitment: Participate in the duty rota once a month, providing timely support during emergencies.

Requirements:
- Degree in social care
- Social Work Wales registration
- 2+ years experience in CWD Social Care
