Social Care Assessor

An exciting opportunity has arisen in Liverpool City Council to join their Neighbourhood Team as a Social Care Assessor (SCA).

This position is:

  • 35 hours per week
  • Initial 3 months

The core responsibilities of this position are:

  • to carry out strength-based assessment/reviews for adults with an impairment who are experiencing difficulties while undertaking activities of everyday life
  • to promote social/physical/mental well being of service users and carers
  • to provide advice, information and support to enable individuals to access universal services

You will achieve the above main areas of responsibilities through the below requirements:

  • Assess need
  • Identify eligibility and outcomes
  • Implement support plans
  • Evaluate/review support plans
  • Have an awareness of capacity, mental health and associated legislation
  • Identify safeguarding concerns

As a Social Care Assessor, you will play a vital role in delivering Adult Social Care to the residents of Liverpool.

By working as part of a team offering the residents of Liverpool strength based assessments under the Care Act, handling a diverse caseload and by working as part of a duty system. You will work within a team of social workers and SCAs and be managed by a supportive group of team leaders and a community manager.
