Occupational Therapist

to work within a Children with Disabilities Team, Children's Safeguarding and Family Support Services.

The post holder will be responsible for the assessment, support planning, and care coordination of children with complex needs within a team and home environment.

DUTIES & RESPONSIBILITIES INCLUDE:

  • To undertake the assessment, support planning and care co-ordination of children with complex needs. Primary tasks to include assessment and recommendation of appropriate equipment and adaptations.
  • To provide advice and support to service users, their families and as appropriate the public on a day to day basis.
  • To promote close and constructive relationships with service users, carers and partner agencies.
  • To identify and highlight resource issues to the Team Leader and Senior Practitioner.
  • To be professionally creative when undertaking the assessment, support planning and care co-ordination for children with complex needs.
  • To use risk creatively to maximise the independence of services users and their families.
  • To be expected to produce complex reports in relation to service users and carers.
  • To be responsible for professional decision making in relation to the assessment, support planning and care co-ordination of children with complex needs.
  • To make difficult or important decisions in partnership with service users and their families as part of the support planning and care co-ordination process.
  • To participate in partnership with Team Leaders and Senior Practitioners in making complex decisions relating to service users and their families. This will include Adult and Children's Safeguarding and risk assessment and planning
  • To be responsible for undertaking a range of tasks including the assessment, support planning and care co-ordination of children with complex needs. This will include the involvement of family members as appropriate.

ESSENTIAL REQUIREMENTS:

  • Educated to Degree level or equivalent in a related field.
  • Appropriate Qualification in Occupational therapy
  • Registered with the Health Professionals Council as an Occupational Therapist
  • Experience of working in a relevant care setting.
  • Person centred thinking and planning
  • Assessment, support planning and care co-ordination
  • Adult and Children's Safeguarding.
  • Understanding of relevant legislation: Community care Act/Mental Capacity Act/Mental Health Act
  • Children's legislation: Disabled children/Personalisation and self-directed support/Transition/Child protection/ Children in Care
