Description

Role Purpose:

To provide needs led support service to older people and people with physical disability or sensory impairment. This will also include service users with mental health needs associated with dementia and on occasions older people who have a functional mental health illness as well as older people with physical frailty. To give time to an allocated group of service users or to make referrals for service users with more complex needs, and thus promote their recovery and maintain their independence in the community. To be responsible for installing, maintaining and removing telecare equipment in service users home setting.

To be responsible for providing an emergency response service or a home visiting service to all service users that have been assessed as needing such a visit and / or providing support and assistance by telephone. To be responsible for providing an emergency response service to all service users connected to the Croydon Careline / Telecare Alarm system.

Key Accountabilities:

To visit service users as assessed and to maintain contact with all service users as outlined in policies and procedures.

  • To ensure that there is a clear written audit trail, in line with policies and procedures.
  • To keep service user records up to date, including conducting service reviews, and updating ARC of any new client details upon installation of telecare equipment or updating of any changes to current client information. This will include notifying ARC of any service users who have ended services.
  • To keep records of calls received and action taken, ensuring that serious or unusual incidents are bought to management team's attention at the earliest opportunity.

Essential knowledge:
? Knowledge of sources of assistance available to adults with care and support needs.
? Knowledge of relevant legislation e.g., Care Act 2014
? An ability to analyse and resolve situations.
? A commitment to empowering & enabling service users.
? An ability to offer informed choices and support to users and carers.
