Responsibilities:

Completing Care Act Assessments to facilitate smooth transitions for service users leaving the hospital.

Supporting discharges from intermediate care bed/inpatient rehab settings.

Collaborating with health colleagues for discharge planning and ensuring the right pathways for individuals.

Promoting a strengths-based approach.

Working closely with individuals, families, carers, and community services (reablement, telecare, care homes, domiciliary home care providers) to promote independence and safety post-hospitalization.

Requirements:

Relevant Level 3 Qualification in Health and Social Care.

Experience in producing quality assessments.

Collaboration with various agencies.

Understanding of the current Discharge to Assess process.
