Adults Senior Social Worker

This role has a starting salary of £46,399 per annum, based on a 36-hour working week.

This is a full-time position but we are happy to consider part-time applications.

For this role you will be based at Surrey Heath Locality Team, Knoll Road, Camberley, Surrey, GU15 3HD however this role is open to hybrid working arrangements. Further information regarding the working pattern can be discussed at interview.

This is a Fixed Term contract/Secondment opportunity up to 31st March 2025. (For internal candidates interested in a Secondment opportunity, please discuss with your Line Manager before submitting your application).

About the Team

Frimley Park is a busy, acute hospital. The Hospital Social Work Team is a dynamic and fast-paced team comprising of Social Workers, Social Care Assistants, and Administrators that work with multiple professionals to ensure the safe discharge and follow up care. The diverse team are committed to making a positive difference for our residents. About the Role

You will have frequent direct contact with the public, visiting our residents and their families on the ward at Frimley Park Hospital, speaking with them on the phone and in some cases visiting our residents in their own home or care setting. Your role will involve a variety of tasks including assessing and reviewing people's needs to promote and expedite safe hospital discharges in collaboration with our health colleagues.

You will work in partnership with individuals, their carers, professional staff in Surrey County Council, other agencies, and service providers in order to support vulnerable adults and their families to identify and arrange services that meet their needs and promote independence, choice and control.

You will have a caseload, working with individuals who need additional support or guidance in order to identify their needs and carrying out risk assessments. You will be involved in supporting safeguarding investigations and attending safeguarding meetings with the support of registered social work practitioners.

You may work with any combination, or all, of the following care groups:

  • Older people
  • People with physical, sensory, cognitive impairments
  • People with mental health problems.

Shortlisting Criteria

To be considered for shortlisting for this position, your application will clearly evidence the following:

  • Substantial experience of working directly with people with social care needs, including completing strength-based and person-centred assessments and visiting them in their own homes or care settings
  • Confidence and competence in working with people with a variety of physical and cognitive impairments
  • Competence in basic IT skills with a willingness to learn how to use a variety of different software systems

Surrey has both urban and rural areas and locality based social care workers will be expected to have a valid driving licence to drive in the UK and access to a vehicle and be willing to travel across a wide geographical area. Reasonable adjustments where needed will be made for successful applicants who have a disability or long-term health condition to enable them to fulfil the requirements of the job.
