Job Description:
Holden Knight Healthcare is excited to represent a Local Council seeking to hire a Senior Social Care Practitioner for their Transition Team. This role offers a fantastic opportunity to join a dynamic team dedicated to supporting young people transitioning from children's to adult services.
As a Senior Social Care Practitioner, you will play a pivotal role in delivering high-quality care assessments and person-centred plans. You will work closely with young people and their families, helping them navigate the transition process smoothly while ensuring that all their care needs are met.
Key Responsibilities:
- Conduct assessments of children and young people transitioning into adult care services.
- Develop and implement person-centred care plans that address individual needs.
- Work collaboratively with healthcare professionals, educational services, and families.
- Support the planning and coordination of care packages to promote independence.
- Offer professional guidance and supervision to less experienced team members.
- Ensure compliance with all relevant legislation and council policies.
Essential Requirements:
- Qualified Social Worker with HCPC registration.
- Experience working within children or adult social care, ideally with a focus on transitions.
- Strong assessment and care planning skills.
- Ability to lead and supervise a small team.
- Excellent communication and interpersonal skills.
