Our client, Herefordshire Council is looking for an Adult Social Worker to join their County Team
Social worker needed for County Team a small, busy, active team based in the centre of Hereford. We work in the main Herefordshire Council offices at Plough Lane two days each week and three days from home. Working from the office gives everyone a chance to catch up, spend time with new staff and support each other in our work.Our legislative framework includes Care Act 2014, Mental Capacity Act 2005, Mental Health Act 1983 enabling us to assess a person's needs and meet them appropriately.
The service users we work with have a range of needs, including Learning Disabilities, Mental Health, Younger and Older Adults with Physical Disabilities, plus housing issues such as hoarding and homelessness. We undertake a wide variety of work including hospital discharges.
The team works closely with the Council's Legal Team to complete Court applications for DoLS Appeals, Community DoLS and Deputyship Applications. We work closely with our local ICB, CHC team, Police, Health colleagues in Primary Health, Mental Health and the Community Learning disability Team. County Team undertake some out of county work, in specialist hospital and community settings, and have the opportunity to work with other multi agency teams out of our area.
Regular supervision takes place each month and you will have an allocated supervisor, who is available on a daily basis, for both formal and informal supervision sessions and case discussions.
