AMHP | Community Mental Health Team | Bracknell Forest Council
We are looking for a Social Worker/Approved Mental Health Professional to work as a care coordinator within an integrated Community Mental Health Team. This is a full time role, based in Bracknell Forest.
Duties:
- Work as an AMHP in a CMHT with an expectation that you will have a caseload and participate in a locality duty rota.
- Complete planned assessments.
- Contribute to service delivery that is person centred and outcome focussed with an emphasis on least restrictive principles and enabling recovery.
- Ensure collaborative work with colleagues and with other professionals and agencies with an emphasis on a multi agency team of support being in place to work with individuals whose needs are complicated or unstable.
To be considered, you must have a Social Work qualification (Diploma in Social Work, Social Work Degree or equivalent) as well as at least 3 years post qualified experience working as a Qualified Social Worker within a similar team.
