Role Overview: As a Qualified Social Worker in our Mental Health & Substance Misuse Services department, you will:
- Conduct comprehensive assessments of service users' needs, risks, and strengths.
- Develop and implement effective care plans in collaboration with service users and their families.
- Provide ongoing support and interventions to promote positive outcomes.
- Collaborate with healthcare professionals, community partners, and external agencies to coordinate integrated care.
- Advocate for the rights and best interests of service users.
- Maintain accurate records and adhere to statutory requirements.
Qualifications:
- Degree in Social Work and registration with the Health and Care Professions Council (HCPC).
- Proven experience working with individuals facing mental health and substance misuse challenges.
- Strong assessment, analytical, and decision-making skills.
- Excellent communication and collaboration abilities.
- Knowledge of relevant legislation, policies, and procedures.
- Commitment to professional standards and ethical practice.
