Role Overview: As a Qualified Social Worker in our Mental Health & Substance Misuse Services department, you will:

  • Conduct comprehensive assessments of service users' needs, risks, and strengths.
  • Develop and implement effective care plans in collaboration with service users and their families.
  • Provide ongoing support and interventions to promote positive outcomes.
  • Collaborate with healthcare professionals, community partners, and external agencies to coordinate integrated care.
  • Advocate for the rights and best interests of service users.
  • Maintain accurate records and adhere to statutory requirements.

Qualifications:

  • Degree in Social Work and registration with the Health and Care Professions Council (HCPC).
  • Proven experience working with individuals facing mental health and substance misuse challenges.
  • Strong assessment, analytical, and decision-making skills.
  • Excellent communication and collaboration abilities.
  • Knowledge of relevant legislation, policies, and procedures.
  • Commitment to professional standards and ethical practice.
