Adult's Social Worker - Transitions Specialist Advisor

About the Role:

Service Care Solutions are seeking a highly skilled and experienced Transitions Specialist Advisor to join Newham Council's team. The successful candidate will be responsible for providing strategic advice as the lead advisor to the Adult Social Care workforce in relation to Transitions and funding arrangements.

Duties:

  • Lead accountability for Transitions and funding arrangements between the Directorate and Children and Young People Services including Education.
  • Oversight of the Adult Social Care staff in completing all relevant documentation with regard to funding for young people transitioning to Adult services.
  • Lead reviews for young people transitioning to Adult Services, using the Carecubed tool (training will be provided) or the Valuing care tool on residents (funded by Newham).
  • Have a pivotal role in strategic input into borough-wide activities with regard to Transitions and funding arrangements.
  • Act as a mentor and point of contact across adult social care in relation to Transitions and ensure that staff have the required skills to complete applications for the Care Funding Panel, the Joint Funding Panel, the CHC Panel and any CYPS panel.
  • Lead negotiations with a range of providers regarding personalisation and cost-effective service delivery to achieve evidence of cost efficiency and cost reductions at a strategic level.
  • Shared responsibility with the Directorate Management Team with regards to the budget provision and allocation of resources relating to Transitions.
  • To ensure the principles of personalisation and strengths-based approach are fully embedded within the practice of the service and that young people are fully supported to have the best possible opportunities and are supported to meet the outcomes they define.
  • Manage highly complex issues within a framework of policy and regulatory guidelines.

About You:

The ideal candidate for this role will have:

  • Social Work qualification/SWE Registration
  • Experience in reviewing residents, ensuring value for money when carrying out reviews and have negotiating skills that will assist when liaising with providers.
  • A proven track record of providing strategic advice in relation to Transitions and funding arrangements.
  • Excellent communication and interpersonal skills, with the ability to mentor and provide guidance to staff across adult social care.
  • Experience in completing applications for the Care Funding Panel, the Joint Funding Panel, the CHC Panel and any CYPS panel.
  • Strong negotiating skills and the ability to achieve evidence of cost efficiency and cost reductions at a strategic level.
  • Experience in managing highly complex issues within a framework of policy and regulatory guidelines.
  • A commitment to ensuring the principles of personalisation and strengths-based approach are fully embedded within the practice of the service.
  • A relevant professional qualification in social work or a related field.
