Key Responsibilities:
- Conduct comprehensive assessments of older individuals' mental health needs.
- Develop and implement care plans to support the mental well-being of clients.
- Provide ongoing support and intervention tailored to individual needs.
- Collaborate with multidisciplinary teams to ensure a holistic approach to care.
Requirements:
- Qualified Social Worker with registration with Social Work England.
- Experience in working with older adults, particularly in mental health.
- Strong commitment to safeguarding and promoting the well-being of vulnerable adults.
- Excellent communication and teamwork skills.
