Social Worker for Children with Disabilities Team

As a Social Worker in the Children with Disabilities Team, you will support children and young people with a range of complex needs, including physical and developmental disabilities. Your role will involve assessing their needs, developing personalised care plans, and coordinating access to services that promote their independence, well-being, and quality of life. This position emphasises providing child-centred support while working closely with families and other professionals to ensure the best outcomes. You will also ensure adherence to statutory requirements under the Children Act 1989 and other relevant legislation focused on safeguarding and promoting the welfare of children with disabilities.

Responsibilities include:

  • Families Act 2014 to understand the needs, risks, and strengths of children with disabilities.
  • Collaborate with families, health professionals, and education providers to gather holistic information
  • Manage a caseload of children with a range of disabilities, ensuring regular review and adjustment of care plans.
  • Identify and respond to any safeguarding concerns, ensuring that children are protected from harm, abuse, or neglect.
  • Advocate on behalf of children and young people to ensure they have a voice in decisions about their care and support
