Role: CHC Case Manager

A great NHS Trust in Lincolnshire is looking for an experienced RGN/RMN/RGLN to join their growing CHC team.
Main Duties:
- Support the Locality Lead in the development of a range of services required for individuals who qualify for packages of care
- Case Managing CHC individuals
- Communicate information, risks, issues and dependencies, including briefings and reports.
- Procide professional nursing advice to acute hospital trusts, care providers and social sercices with regards to Continuing Healthcare.
Minumum Qualifications:
- Registered Nurse RGN, RMN or RNLD or Allied Health Professional with qualification
- Knowledge of NHS and social services guidance
- Strong knowledge base around safeguarding children and Adults
- Evidence of teaching/mentorship
- Clinical knowledge base in order to ensure comprehensive holistic assessment for continuing care
- Must have a UK driving Licence as travel county-wide is required at times.
