Social Worker - Referral and Assessment Team

We are recruiting a Social Worker - Referral and Assessment Team across Kingston and Richmond.

This is a full time 36 hours per week role and additional hours may be required to meet the needs of the service.

Social Worker Pay Grade is £39,951 -£40,614 Senior Social Worker Pay Grade is £46,041 - £49,083.

Job Details -

  • Provide comprehensive social work assessments to adults with complex needs, including those with mental health issues, disabilities, and older adults requiring support.
  • Develop and implement care plans tailored to individual service users, in collaboration with the wider multidisciplinary team and relevant stakeholders.
  • Offer practical advice, advocacy, and support to service users and their families, empowering them to make informed decisions about their care and wellbeing.
  • Liaise effectively with external agencies, including health professionals, housing authorities, and community support services, to coordinate holistic care packages and ensure continuity of support.
  • Maintain accurate and up-to-date records in accordance with legal and regulatory requirements, ensuring confidentiality and data protection standards are upheld.

Requirements -

  • Qualified Social Worker with current registration with Social Work England.
  • Proven experience working within adult social care settings, ideally within a multi-disciplinary team environment.
