Care Coordinator/Manager

We are seeking for a detail-oriented and compassionate Homecare Care Coordinator to join our dynamic team. The successful candidate will play a vital role in managing and coordinating homecare services, ensuring our Service Users receive the highest level of care tailored to their specific needs.

Key Responsibilities:

  • Allocating Personal Care Assistants with the right skills and experience to deliver the best quality care and support to each Service User.
  • Scheduling/organising Personal Care Assistants' rota to minimise changes to the service and travel in between Service User's homes.
  • Responding efficiently to day-to-day changes in the care and support packages.
  • Coordinating, planning, and managing adequate staff cover and ensuring that changes are reflected as soon as possible on the rostering system.
  • Maintaining a good level of communication with all relevant parties involved in the provision of Service User's care.
  • Providing exceptional customer service to our Personal Care Assistants, Service Users and other professionals.

Qualification/Experience/Skills Requirements:

  • Previous coordinating skills within the domiciliary care sector are essential (Minimum 2 years).
  • QCF Level 3 in Health and Social Care (or an equivalent qualification).
  • Strong interpersonal and communication abilities.
  • Full driving license and access to own vehicle.
  • Satisfactory pre-employment checks (Criminal History, References and Right to Work in the UK)
  • Proficiency in using rostering software (Care Planner and Log My Care).
  • Excellent organisational and time management skills.
  • Ability to work independently and as part of a team.
