Our Client, Nottingham City, is seeking an Adult's Social Worker to join their Discharge to Assess Team.

Responsibilities of this role:
- This role is set within an Discharge to Assess Team and will cover the Hospitals in the South of Nottinghamshire (Queen's medical centre and City Hospital).
- The candidate will need to have one or more of the following qualifications AMPH, BIA or Practice Educator (full award)
Requirements of this role:
- Must be a Qualified Social Worker, registered with Social Work England.
- Must have minimum of 2 years' post qualified experience.
