Care Coordinator
Our client, a well-established, family-run care service based in Colchester, is seeking a dedicated Care Coordinator to join their team. Salary: £23,000 - £26,000 annually, dependent on experience.
Role Purpose: The Care Coordinator will be responsible for ensuring that clients receive the highest quality care and support within their own homes. This involves effectively coordinating care services through strong organizational skills, excellent communication, and a supportive demeanor.
Key Responsibilities:
- Manage and optimize staff schedules to deliver seamless care.
- Match Care Assistants to clients based on individual needs and preferences.
- Collaborate with management to maintain a skilled workforce and provide necessary coverage.
- Process new care referrals and updates to care plans promptly and accurately.
Essential Requirements:
- Proven experience in care coordination and teamwork.
- Proficiency in using IT systems for scheduling and administration.
- Flexibility to participate in an on-call rota covering evenings and weekends.
Working Hours: Monday to Friday, 8:30am - 5:00pm, with additional on-call duties as required.
