Responsibilities:
- The primary function of the team is to undertake reassessments of need under the Care Act and s117, for people who have diagnosed severe and enduring mental illness, who are under psychiatry and in receipt of commissioned package support
- Element of arranging services where there has been a change in need and also working with external agencies/teams in the event the person were to relapse
To be considered for the Social Worker role:
- 6 months-3 years experience of working within a similar mental health/adult social work setting
- Diploma/Degree in Social Work
- Social Work England registration
- Experience of undertaking assessments under the care act and commissioning services
- Knowledge and experience of working with secondary mental health pathways
- Qualified AMHPS also welcome, as may be able to support the AMHP rota.
