**Adults Social Worker
Blackburn & Darwen looking for an experienced Adults Social Worker to join their growing team in the Community Mental Health Team based at Daisyfield Mill. As a social worker, you will be working closely with both health and social care staff to support individuals with serious and enduring mental health conditions, complex care & support needs, including support to their families and carers with the aim to improve mental health and wellbeing and to promote recovery and social inclusion.
**Your Responsibilities
- Complete high-quality assessments and care and support plans
- Implement the Care Act 2014
- Manage a realistic caseload using job planning
- Work autonomously and as part of a multi-disciplinary team ensuring the best outcomes for residents of Blackburn with Darwen
- Be flexible and responsive to the people you support and your colleagues
**Your Qualifications and Skills
- Registered as a Social Worker with Social Work England
- At least 2 years plus mental health social work experience, 3 years plus desirable
- Good working knowledge of the Care Act, Mental Capacity, Safeguarding, Mental Health Act
- Clean driving licence and access to a car
