We are seeking a dedicated Case Management Worker to join our team at a homelessness hostel in Liverpool. This role is crucial in supporting service users with a range of challenges, including offending behaviour, mental health issues, substance misuse, and rough sleeping. The ideal candidate will have a passion for making a difference in the lives of vulnerable individuals and the resilience to work in a demanding yet rewarding environment.

Day-to-day of the role:

  • Conduct comprehensive assessments of service users' needs and risks.
  • Develop and implement personalised support plans to assist service users in addressing their challenges and achieving their goals.
  • Provide one-to-one support and group work sessions to service users, focusing on areas such as life skills, tenancy sustainment, and accessing community resources.
  • Work collaboratively with external agencies, including mental health services, substance misuse services, and the criminal justice system, to coordinate support for service users.
  • Maintain accurate and confidential case management records and reports.
  • Regularly review and update support plans to reflect service users' progress and changing needs.
  • Engage with service users to encourage and empower them to take an active role in their recovery and reintegration into the community.
  • Provide crisis intervention as needed and contribute to the hostel's overall safety and security.

Required Skills & Qualifications:

  • Experience in a similar role, preferably within a homelessness service or related field.
  • Knowledge of the issues faced by individuals with offending behaviour, mental health challenges, substance misuse, and rough sleeping.
  • Strong interpersonal and communication skills, with the ability to build rapport and trust with service users.
  • Ability to work effectively as part of a team and independently.
  • Resilience and adaptability to work in a challenging and fast-paced environment.
  • Commitment to promoting the dignity, rights, and well-being of service users.
  • Proficiency in case management and record-keeping.
