£15.40 per hour (£29,627).

Exciting opportunity to work with one of my local social housing clients.

The role

To provide responsive, high quality, support to clients, in order to promote independent living, as appropriate; clients
can be defined as individuals, families or children that might have a range of diverse needs.

+ To support clients, in a flexible and person centred way, utilising PSI interventions and delivering practical tasks,
which address their needs and aspirations, through implementing and authorising support/ treatment plans.
+ To undertake, authorise and review individual client needs assessment which generates an Outcome Star which
shows the distance travelled and outcomes achieved by the clients.
+ To undertake, authorise and review individual client risk assessments through effective communication and
rapport building with the client.
+ To be accountable for adherence to internal targets and performance measures.

Experience needed

+ Experience of empowering clients to build and develop strong support networks and positive relationships
+ A good understanding of Safeguarding
+ A proven ability to work with clients to successfully exit the service and optimising their ability
+ A proven ability to work with individuals with diverse needs
+ Experience of working across a range of sectors
