Clinical Case Manager

A forward-thinking, quality focused independent case management company, specialising in catastrophic and complex injuries is looking for a Clinical Case Manager to join the Children and Young People team.

We are open to applicants who wish to have the benefits of working on a self-employed basis, or who are looking for an employed role.

Along with a salary of up to £56,000 per annum (depending on experience), you will also receive a company car and enhanced annual leave (employed staff only), access to an employee assistance and wellbeing programme, regular in-house training days and support of an administration team and a CQC Registered Manager.

What you'll do as Clinical Case Manager?

As CYP Clinical Case Manager, you will provide support to children and young people with a range of needs including traumatic brain injury, acquired brain injury, spinal cord injury and amputation. You should be highly competent and dynamic in developing client-centred rehabilitation packages.

Some elements of the role can be completed remotely; however, this position also requires regular ‘real world' face-to-face meetings with clients and their families, support workers and other members of the multi-disciplinary teams as well as some occasional attendance at the main office in Ipswich.

Staff development is supported with in-house training days and mentoring.

Essential Requirements

  • Must have a background in Occupational Therapy, Nursing, Physiotherapy, Speech and Language Therapy, Social Work or Clinical Psychology
  • Registration with HCPC, NMC, or Social Work England
  • Experience of working with children and young people in a clinical capacity
  • Be able to demonstrate an ability to support children and young people to have the best opportunities and outcomes
  • Car driver
  • Proactive, ability to take initiative and work independently
  • Excellent organisational, communication and computer skills
  • Competent in developing bespoke rehabilitation packages
  • Ability to implement and supervise a variety of clinical and non-clinical services
  • Good time management with adherence to deadlines
  • Excellent report writing and record keeping skills
  • A knowledge of safeguarding.

Desirable Skills

  • Experience in a similar case management role
  • Knowledge of ‘iinsight' the cloud-based case management and reporting system
  • Advanced BABICM membership
  • Experience in care planning and managing care teams
  • An understanding of statutory funding.

The main office is based in Ipswich, Suffolk and we are particularly interested in applicants based in East Anglia (Suffolk, Essex, Cambridgeshire & Norfolk).
