We are collaborating with an NHS Trust who are seeking a Lead Complex Case Manager to support their Continuing Healthcare Team for a 6 month initial contract.
Lead Complex Case Manager
Hybrid working: Patient facing in Birmingham & Solihull / remote
£300 - £320 per day (Inside IR35)
37.5 hours a week
6 month initial contract
You will conduct comprehensive Continuing Healthcare and Funded Nurse Care assessments for adults over 18 with complex health needs, including assessment, review, and monitoring for those funded by NHS CHC. This includes retrospective and appeal cases. You will act as a Case Manager for clients in the ICB, assessing needs, planning and evaluating service provision to ensure optimal function and appropriate care in the best environment.
Requirements:
- Registered Learning Disability Nurse / RGN with a valid NMC Pin
- Previous experience of undertaking Continuing Care Assessments
- Significant experience of working with adults and young people with complex and long-term health needs, disabilities and those with life limited/palliative or end of life
- Working Knowledge of DOLS
- Working knowledge of the personaliation and SEND agenda
