Responsibilities:

  • Provide holistic assessments and support plans for adults undergoing transitional phases, such as leaving care, entering adulthood, or transitioning between services.
  • Work closely with young adults, their families, and relevant agencies to facilitate smooth transitions and promote independence and well-being.
  • Advocate for the rights and preferences of service users, ensuring their voices are heard and respected throughout the transition process.
  • Collaborate with multi-disciplinary teams to coordinate support services and resources tailored to the unique needs of each individual.
  • Offer guidance and practical assistance to help adults develop essential life skills, including budgeting, housing, education, employment, and social integration.
  • Monitor and review transition plans regularly, adjusting them as necessary to reflect changing circumstances and goals.
  • Provide emotional support and practical advice to help adults cope with the challenges and uncertainties associated with transitions.

Requirements:

  • Qualified Social Worker with current registration with Social Work England.
  • Proven experience working with adults undergoing transitions, preferably within a statutory setting.
  • Knowledge of relevant legislation, policies, and procedures related to adult transitions and support services.
  • Strong assessment, communication, and interpersonal skills, with the ability to engage effectively with individuals and families from diverse backgrounds.
  • Ability to work collaboratively with stakeholders, including young adults, families, colleagues, and partner agencies.
  • Empathy, patience, and a non-judgmental approach to supporting individuals through periods of change and uncertainty.
  • Commitment to continuous professional development and reflective practice.
