Support Coordinator Worker Required

Venn Group are looking for a Support Coordinator to work for a housing association in the Southwest
The successful candidate will be responsible for:
- Performing outreach community work to rough sleepers
- Managing a caseload of customers
- Developing and implementing support plans
- Liaising with external agencies such as probation services
- Supporting service users manage physical and mental health
- Providing Tenancy support
- Offering assistance in money management
The successful candidate will have:
- Enhanced DBS
- Driving Licence
