Prison Link Worker- HMP High Down, HMP Wandsworth and HMP Brixton

Job Summary:

As a Prison Link Worker, you will be part of a flexible, multi-disciplinary team working across a cluster of delivery sites, primarily within custodial settings across London. Your role will involve providing critical support to individuals with substance misuse issues, both pre and post-release, ensuring smooth transitions and continuous care from prison to the community.

Location: HMP High Down, HMP Wandsworth and HMP Brixton

Work hours: 35 hours per week

Salary: £25,154.00 - £30,742.00

Main Duties and Responsibilities:

  • Build and maintain relationships with individuals in prison to maximize engagement post-release.
  • Develop and implement comprehensive release plans in collaboration with prison-based teams.
  • Provide meet-and-greet services, supporting individuals during the first 24 hours post-release and facilitating their connection to treatment and support resources.
  • Deliver 1:1 sessions and group work, both face-to-face and virtually.
  • Complete risk assessments, action plans, session reports, and maintain accurate records.
  • Collaborate with prison substance misuse and healthcare teams to ensure a robust post-release pathway.
  • Meet all contractual service level agreements and key performance targets.
  • Manage caseload effectively, ensuring prioritization and timely support for service users.
  • Proactively contribute to the overall smooth running of the service.
  • Adhere to all safeguarding policies, reporting concerns promptly.
  • Maintain accurate and confidential service user records.
  • Participate in multi-disciplinary reviews and team meetings.
  • Ensure compliance with all relevant legal, regulatory, and ethical standards.
  • Ensure accurate and timely data entry into systems.
  • Maintain high standards of case management documentation.
  • Attend training sessions and actively engage in professional development.

Person Specification:

  • Understanding the needs of clients upon release and knowledge of addiction approaches.
  • Experience managing high caseloads, conducting risk assessments, and creating SMART action plans.
  • Experience delivering 1:1 and group-work interventions using motivational techniques.
  • Ability to work effectively with key partners and build strong relationships.
  • Strong organizational, time management, and communication skills.
  • Driving license and access to a car (required for visits to prisons and meet-and-greet support).

Desirable:

  • Personal experience of addiction or imprisonment (with relevant clean time or release period).
  • Experience providing counselling services or working with those with health and wellbeing or substance misuse issues.
  • Knowledge of the 12-step recovery process and recovery agenda.
  • Qualifications in substance misuse, advice and guidance, counselling, or PTTLLS.

A current DBS is required for this role, which we can process for you and HMPPS Enhanced Level 1 vetting.
