Integrated Discharge Social Worker

We are looking for dedicated and experienced social workers to join our Intergrated Discharge team in South Yorkshire.

Job title: Social Worker, Integrated Discharge Team

Job Type: Full time, Permanent

Hours: 37 hours per week (7 day working as part of a rota basis)

Salary: £31,372-£41,483 per annum

Purpose:

To assess a person's ability to manage safely with daily tasks, supporting their hospital discharge. This typically involves working together with community and hospital-based services.

Responsibilities and Duties:

To be person-centred and reinforce a patient's choice and control over their care and support.

To promote a patient's well-being throughout their journey in the acute and community setting.

To provide strength-based assessments and independence plans for patients who are eligible for social care services.

To complete Care Act 2014 compliant assessments and reviews for patients who have ongoing or complex needs.

To source provisions both formal and informal in support of patient needs, such as care packages, equipment, adaptations, reablement, or voluntary sector services.

To interact with a plethora of professionals daily, such as nurses, therapists, discharge coordinators, GPs, community teams, and care providers.

To guide and support colleagues from other professions or colleagues less experienced in social care processes.

Knowledge & Skills:

Knowledge of related legislation, policy, and frameworks, such as the Mental Capacity Act 2005, the Deprivation of Liberty Safeguards, the Safeguarding Adults Policy, and the Hospital Discharge Service Requirements 2020.

You need to be able to communicate effectively with patients, carers, colleagues, and other professionals, using different methods and styles depending on the situation and the audience.

To be able to listen actively, empathise, and build rapport with the people you work with.

To be able to identify and resolve issues that may arise during the discharge process, such as delays, barriers, conflicts, or complaints.

To be able to manage your time and workload effectively, prioritising tasks and meeting deadlines

Experience:

Experience of safeguarding processes and procedures, such as identifying and reporting abuse or neglect, making referrals, and participating in safeguarding enquiries or reviews.

Relevant post qualification experience within adults' services.

Qualifications:

CQSW / DIPSW / Degree in Social Work or equivalent.
