Key Responsibilities:
- Conduct assessments for highly complex patients within the Continuing Healthcare Team across the CTM Health board.
- Perform evaluations in patients' homes to determine the need for specialist manual handling equipment or techniques.
- Submit equipment requests to the CHC Panel for review and approval.
- Develop and implement tailored intervention plans for patients based on assessment findings.
- Collaborate with multidisciplinary teams to ensure integrated and effective patient care.
Qualifications:
- Registered Occupational Therapist with a valid HCPC registration.
- Proven experience in managing complex cases and providing high-quality occupational therapy services.
- Strong knowledge of manual handling techniques and equipment.
- Excellent communication skills and the ability to work effectively with patients, families, and healthcare professionals.
Desirable Skills:
- Experience working within a Continuing Healthcare Team or similar setting.
- Familiarity with the CTM Health board's policies and procedures.
