Position: Looked After and Permanence Team Member

Location: Durham, United Kingdom

Job Type: Contract/Temporary (with possibility of extension)

About the Role:

As a Looked After and Permanence Team Member, you will play a crucial role in supporting children and young people who are in the care system or in need of permanence solutions. You will work closely with social workers, foster carers, adoptive families, and other professionals to ensure the well-being and stability of these vulnerable individuals.

Key Responsibilities:

  • Collaborate with social workers to assess the needs of children and young people in care and develop appropriate care plans.
  • Support the matching and placement process for children and young people requiring foster care or adoption.
  • Coordinate and participate in family finding activities to identify suitable permanent placements.
  • Provide ongoing support and guidance to foster carers and adoptive families, ensuring their needs are met.
  • Conduct regular visits and reviews to assess the progress and well-being of children and young people.
  • Advocate for the rights and best interests of children and young people within the care system.
  • Liaise with external agencies and professionals to access additional support and services when required.
  • Maintain accurate and up-to-date records, case files, and documentation.

Requirements:

  • A relevant qualification in social work, social care, or a related field (e.g., Diploma in Social Work, BA in Social Care).
  • Proven experience in working with looked after children, care leavers, or adoption services.
  • Sound knowledge of relevant legislation and policies surrounding looked after children and permanence.
  • Strong assessment and care planning skills, with the ability to work collaboratively with multi-disciplinary teams.
  • Excellent communication, interpersonal, and relationship-building skills.
  • Ability to manage and prioritise a demanding workload, while maintaining attention to detail.
  • Flexibility to adapt to changing situations and work effectively under pressure.
  • Proficiency in using relevant IT systems and software for recording and reporting purposes.
