- Newly Established, Specialised Team: This recently established team is specifically designed to address the unique needs of young people transitioning into Adult Services. Be a pioneer in shaping the future of adult care services for individuals with Learning Disabilities, Autism, and neurodiversity, making a direct impact on the community.
- Manageable Caseload: Handle an average of 20 cases, carefully balanced to match your expertise and the complexity of each case. Focus on delivering high-quality care without being overwhelmed, ensuring you can provide thorough and effective support to each individual.

Requirements:
- Degree in social care
- Social Work England registration
- 2+ years experience in Learning Disabilities and/or Transition Social Work
