Lead with Purpose: Become a Weekend Deputy Team Manager in Southwark's Hospital Discharge Transfers of Care Unit!

Are you ready to take on a leadership role that makes a real difference, even outside the usual 9-to-5? Southwark Council is seeking an energetic and passionate Deputy Team Manager to oversee our Hospital Discharge Transfers of Care Unit during critical weekend periods. This unique role, with a 36-hour workweek spanning Friday to Tuesday, offers the perfect opportunity to lead a dedicated team while ensuring that patient care and safe discharges continue smoothly through the weekend.

The Hospital Discharge Transfers of Care Unit is at the heart of Southwark's commitment to providing seamless and effective care transitions from hospital to home or other care settings. Our team is dedicated to managing the complexities of hospital discharges, safeguarding concerns, and care planning with precision and compassion. As a leader during the weekend, you will ensure that services remain robust, responsive, and focused on the needs of our residents even during peak periods.

About the Role

In this pivotal role, you'll lead and inspire your team of social workers and healthcare professionals to deliver high-quality care every day, especially during the busy weekend periods:

  • Operational Leadership: Oversee the day-to-day operations from Friday to Tuesday, ensuring social workers efficiently triage referrals, allocate cases, and authorise assessments, keeping the flow of care moving smoothly.
  • Guidance and Support: Provide expert guidance on complex discharges, safeguarding issues, and mental capacity assessments, ensuring your team is well-supported and empowered to make informed decisions.
  • Collaborative Networking: Serve as the lead manager for weekend hospital discharges at Kings and St Thomas Hospital, ensuring that care plans are maintained, and discharges are handled effectively. You'll also be the key link for managing step-down flats in extra care schemes, ensuring that referrals are triaged and residents are placed appropriately.
  • Team Development: Mentor and supervise social workers and assistant practitioners, fostering an environment of continuous learning and reflective practice, ensuring that your team can operate autonomously and confidently.

About You

We are looking for a dynamic, experienced leader with a strong commitment to social care excellence. The ideal candidate will bring:

  • Deep Knowledge: A solid understanding of the Care Act, Mental Capacity Act, and Continuing Healthcare processes, with a track record of applying these frameworks in practice.
  • Proven Experience: Experience in leading MDT and safeguarding meetings, providing reflective supervision, and supporting the professional development of others. Experience as a Practice Educator or within ASYE is desirable but not essential—what matters most is your commitment to growth and development.
  • Leadership Skills: The ability to lead with confidence, think creatively under pressure, and make decisive, informed decisions that ensure the best outcomes for residents.
