Location: Cardiff, CF5 and CF64
Contract length: Until January 2025 and a chance of extension from there
Pay Rate: £16.00 per hour umbrella
Working Hours: 37 hours per week, Monday to Friday
We have one role to work in a Day Centre for Adults with Learning disabilities. And another wot work at Llandough hospital, This role will consist mainly of completing reviews / assessments under the Social Services and Well-being Act.
Duties include:
- To arrange and monitor appropriate services in line with individual plans.
- To provide advice and practical help.
- To liaise with voluntary and statutory agencies.
- To assist the case managers in the team in the carrying out of specific tasks
- To participate in team meetings and duty rotas
- To undertake these duties in line with County Policies and Procedures.
- To undertake training and self-development and to contribute to this process in others.
