Shift Pattern: 37.5 hours per week, Monday to Friday
Pay rate: £19.38

Main duties of the job
- Assessment of patients on reablement unit including mobility, functional assessments and cognitive assessments.
- Assessing for and ordering of appropriate equipment.
- Delegation of appropriate work to Therapy Practitioners.
- Multidisciplinary working with physiotherapy and nursing staff.
- Discharge planning including liaising with relatives and external agencies.
- Completing timely referrals to appropriate teams to facilitate discharges.
To be successful in applying for this role you will need:
- Current relevant professional registration (HCPC Registration)
- Minimum 2 years post-qualification experience (Occupational Therapy Degree/Physiotherapy Degree)
- Enhanced DBS
- Experience of working within the NHS
In addition to experience, all Qualified roles will be required to provide evidence of
- Relevant Qualifications
- Registration with the relevant Registration Body if applicable.
