About the team

This is an exciting opportunity to join Hertfordshire's Community Learning Disability Nursing service in Hertfordshire County Council. As a Band 5 Nurse you will work within the Adult Disability Team to reduce health inequalities for people living in the Watford and Three Rivers locality.

About the role

As a Community Nurse you will work alongside paid and family carers to support people with learning disabilities to access mainstream health services and support clinicians to make reasonable adjustments. Your skills in adapting and creating the right approach for the individual will help bridge the gaps and reduce the risk of avoidable health conditions.

About you

Applicants must have knowledge of the requirements and challenges of community nursing and be able to demonstrate transferrable skills from other nursing environments if you have not had community experience. You must be registered as a Nurse with a valid NMC registration and if your registration is not in Learning Disability Nursing be able to demonstrate experience, skills and knowledge in working in this area.

As the post is in the community, the ability to drive and have access to a vehicle is an essential requirement.
