We have two exciting opportunities available, each managing a different team within our innovative care services. Whether you are drawn to the fast-paced environment of hospital discharge or the pioneering work in rehabilitation and reablement, Southwark offers a role that will align with your strengths and aspirations.

About the Transfer of Care Teams

Our Transfer of Care Services are dedicated to ensuring smooth and effective transitions for individuals moving from hospital settings back to their homes or other care environments. We operate two distinct units within this service, each with its own focus and challenges:

- Hospital Discharge Transfers of Care Unit (TOCN): This team is at the forefront of managing complex hospital discharges. The unit plays a critical role in ensuring that our most vulnerable residents receive timely and appropriate care as they transition from the hospital to their next stage of care, whether that be at home or in another care setting. The work is fast-paced, requiring quick decision-making, strong collaboration with NHS Trusts, and a deep understanding of safeguarding and care planning.

In this role, you'll lead a team that manages the complexities of hospital discharges, ensuring that every transition is handled with care and precision. Your leadership will be crucial in triaging referrals, managing safeguarding concerns, and authorising assessments. You'll also be the key point of contact for system escalations and will work closely with hospital teams to align expectations and deliver the best outcomes for residents.

- Transfers of Care Avon Unit (TOCBB): The Avon Unit represents a cutting-edge approach to care, specialising in Discharge to Assess (D2A) and rehabilitation services. This team brings together social workers, occupational therapists, and physiotherapists to support residents in regaining their independence post-hospitalisation. The Avon Unit is a hub of innovation, where creative solutions and a strengths-based approach are at the core of the service.

As the Deputy Team Manager of the Avon Unit, you will be at the helm of a team that is driving innovation in rehabilitation and reablement. This role involves leading interdisciplinary teams, setting ambitious goals, and implementing creative solutions that enhance the independence and well-being of our residents. Your leadership will also involve mentoring staff, managing complex safeguarding cases, and fostering a collaborative environment with care home and NHS partners.

As a Deputy Team Manager, you will have the opportunity to:

  • Lead with Vision: Oversee the daily operations of your team, ensuring that our services are delivered with the highest standards of care and efficiency.
  • Inspire and Support Your Team: Provide expert guidance on complex cases, mentor your team members, and foster a culture of continuous professional development and reflective practice.
  • Drive Collaboration and Innovation: Work closely with NHS Trust colleagues and other partners to ensure seamless transitions and to continuously improve the services we provide.

About You

We are looking for leaders who are passionate about social care and committed to excellence. You should bring:

  • In-Depth Knowledge: A thorough understanding of the Care Act, Mental Capacity Act, and Continuing Healthcare processes, with the ability to apply these frameworks in practice.
  • Proven Leadership Experience: Experience in leading multidisciplinary teams, providing reflective supervision, and supporting the professional growth of others. A background as a Practice Educator or within ASYE is a plus.
  • Innovative Thinking: The ability to think creatively, act decisively, and inspire your team to achieve outstanding results.
