Job Title: Adults Social Worker - Neighbourhood Team East
Location: Hebburn, South Tyneside
Salary: £24 - £26 per hour
We are seeking a dedicated and experienced Adults Social Worker to join our Neighbourhood Team East in Hebburn. This role involves working with adults in the community, providing support and interventions to promote independence, well-being, and quality of life.
Responsibilities:
- Manage a diverse caseload of adults, conducting comprehensive assessments to determine their needs and develop personalised care plans.
- Provide support and interventions to individuals to enhance their independence and quality of life.
- Conduct face-to-face visits and maintain regular contact with service users, families, and carers.
- Collaborate with other professionals, including health services, housing, and voluntary organisations, to provide holistic support.
- Monitor and review care plans, making adjustments as necessary to ensure they continue to meet the needs of service users.
- Advocate for the rights and needs of adults, ensuring their voices are heard and respected.
- Maintain accurate and up-to-date records and reports in compliance with statutory requirements and best practices.
Requirements:
- Registered with Social work England
- A Degree in Social work or Diploma in SW, CQSW, CSS
- Right to Work in UK
- Enhanced DBS check, dated within the last 12 months
- A full UK driving licence
- Minimum of two years post-qualifying experience in adult social work.
- Significant experience in adult social work, with a strong understanding of the needs and challenges faced by adults in the community.
- Knowledge of relevant legislation, policies, and procedures.
